Iran has released rare footage of Supreme Leader Mojtaba Khamenei, in what appears to be an effort to counter growing speculation about his health and prolonged absence from public view.

The footage, broadcast by Iran’s semi-official Mehr News Agency on Sunday, shows Khamenei meeting with several people and apparently addressing them. The report did not say when or where the footage was recorded, or identify the people accompanying him.

The release came as questions over the Supreme Leader’s health intensified following months without a public appearance since he succeeded his father, Ayatollah Ali Khamenei, in March.

Iranian authorities have provided limited information about his condition since he took office. State media previously reported that he was seriously injured in the February 28 US-Israeli strike that killed his father, while his continued absence has fuelled speculation about the extent of his injuries and his ability to carry out his duties.

The newly released footage does not establish when it was recorded and therefore does not independently confirm Khamenei’s current health.

Its publication, however, comes alongside renewed efforts by Iranian officials to demonstrate that the 56-year-old leader remains involved in state affairs.

Iranian state media reported on Sunday that President Masoud Pezeshkian had recently met Khamenei, with the meeting taking place in late July. The two discussed military affairs and economic issues, including resource management, energy use, foreign exchange and economic relations with foreign partners, according to the reports.

The meeting was the second publicly reported encounter between Pezeshkian and Khamenei since the latter assumed the supreme leadership. A previous meeting was reported in May.

The reports have emerged against a backdrop of conflicting statements from Pezeshkian about his access to the Supreme Leader. In July, the president said his interactions with Khamenei were increasing, but in early August described communication with him as difficult.

The uncertainty surrounding Khamenei’s visibility has also drawn attention from senior Iranian officials.

Basij Organisation deputy Qasem Qoraishi said on Sunday that further footage showing Khamenei among the public and meeting commanders of the armed forces would be released, apparently in response to speculation about his health and absence from public life.

Khamenei became Supreme Leader after the death of his father in the opening hours of the US-Israeli attack on Iran on February 28. Iran’s Assembly of Experts selected him as his father’s successor on March 8, making him the third Supreme Leader of the Islamic Republic.

His succession marked the first transfer of Iran’s supreme leadership from father to son since the 1979 Islamic Revolution and placed him at the centre of the country’s leadership during an unprecedented period of conflict with the United States and Israel.

Khamenei has maintained a notably low public profile since taking office. He was absent from the major funeral ceremonies for his father in July, further intensifying questions about his whereabouts and health.

The renewed attention comes as Iran continues to navigate the consequences of its military confrontation with the United States and the uncertain diplomatic efforts that followed.

The two countries reached an interim understanding in June intended to halt military operations and create space for negotiations towards a longer-term arrangement. The agreement has since come under strain, with disputes over its implementation and the status of the Strait of Hormuz continuing to complicate relations.

The Strait has become one of the central points of the confrontation, with Iran retaining significant leverage over a waterway critical to global energy supplies. Tehran has linked its position on reopening the route to wider demands concerning the US and the terms of any future settlement.

Against that backdrop, the appearance of Mojtaba Khamenei in newly released footage carries significance beyond the question of his health.

For Tehran, demonstrating that the Supreme Leader is able to meet officials and engage with matters of state may help reinforce confidence in the continuity of Iran’s leadership at a time of heightened political and military uncertainty.

But until the footage is independently dated and further evidence of Khamenei’s current activities emerges, questions over his health and the extent of his public role are likely to remain.
